GENERAL INFORMATION FORM.
Before coming to camp meeting, download and fill out the YOUTH CONSENT FORM: Registration, Medical Release & Information Form. This is a parental consent form that must be completed by the appropriate adult. this form provides our camp counselors with a record of a youth's physician, health insurance information and other vital parental contact information.
